Area resources such as SRMC, PPHD, Economic Development and others will use your responses to help identify the most pressing concerns. Thank you for your feedback!
Every three years SRMC asks for your support as we assess the health needs of our community through a process known as a Community Health Needs Assessment (CHNA). It is our goal at Sidney Regional Medical Center, a nonprofit critical access healthcare organization, to stay focused on the needs of those we serve.

We hope you will take the next several minutes and provide feedback regarding the health of our community. This survey is completely confidential and designed to give direction about how our organization can make an impact by creating programs and partnerships that will positively improve key areas that are of concern.
