The purpose of this survey is to provide an overview of the fourth goal of Oregon's Viral Hepatitis Elimination Plan,  Improve Surveillance and Data Usage, and solicit feedback from a wide variety of partners.

For the goal of improving surveillance and data usage, we propose three main objectives that represent changes, outcomes and impacts that we want to achieve:

1) Monitor control and spread of viral hepatitis
2) Monitor racial and ethnic disparities, and
3) Monitor morbidity and mortality due to viral hepatitis and track access to testing and treatment

We would like you to see what we plan to do over the next 3-5 years and solicit your opinions on our proposal.  We would also like to hear your suggestions on other data you would like to see and the best way for you to access hepatitis information. As you go through the document, we have boxes at the end of each set of strategies where we would love your feedback on each aspect.
