Consent Form
 
Project Title: AACTE Women in Leadership  - Disrupting Inequities, Beliefs and Experiences
Principal Investigator: Beth Kubitskey, Associate Dean COE
 
Purpose: The purpose of this study is to better understand the challenges and contributions of women in leadership or aspiring to be leaders. 
 
Study Procedures: Participation in this study involves completing an online survey.  The survey should take up to 10 minutes.   
 
Types of Data Collected: We will ask short questions about your experience as a leader and in leadership situations with respect to disrupting inequities. 
 
Risks: The primary risk of participation in this study is a potential loss of confidentiality.   Some of the survey questions are personal in nature and may make you feel uncomfortable. You do not have to answer any questions that make you uncomfortable or that you do not want to answer.
 
Benefits: You will not directly benefit from participating in this research. Benefits to leadership community may be understanding and/or disrupting inequity. 
 
Confidentiality: We will keep your information confidential by never collecting identifiable data. The survey results sent to us will be void of any identifiable data.
 
The principal investigator and the research team will have access to the anonymous information you provide for research purposes only. We may share your anonymous information with other researchers outside of Eastern Michigan University. We will have no identifiable information.
 
The data exported from SurveyMonkey will contain no identifiable information and will be stored on a password-protected computer.  Transfer of data will be through password protected email and will also not contain any identifiable data.  The data will be stored indefinitely. 
 
The results of this research may be published or used for teaching. There is no identifiable information.

Voluntary participation
 
Participation in this research study is your choice. You may refuse to participate at any time. However, once the survey data is submitted it will be impossible to identify which data are yours.
