2019 APH Customer Satisfaction Survey

Thanks so much for sharing your thoughts about APH products through our Customer Satisfaction Survey. By responding to this brief 11-question survey, you are helping us improve our products and our product development.

After each question that asks for a rating, you have the opportunity to tell us why you chose that rating. There you can reflect your experience with and suggestions for APH products and services. There is also an open-ended item, #11, that provides an opportunity for you to voice additional suggestions, comments, and experiences with APH products and services.

This survey is also conducted to collect APH performance data as required by the federal Government Performance and Results Act (GPRA). The resulting data will be summarized and shared with the U.S. Department of Education.

All responses will remain confidential.

Note: This survey is being conducted using a web-based survey administration service with a proven history of accessibility. The needs of screen reader users were kept in mind as we designed the survey, but individual computer systems vary widely in the ways they perform. If you are completing this survey with a screen reader, you may notice places where text is repeated. This is caused by the survey software and should be ignored. EXPLANATION: Ex Officio Trustees (EOTs) are appointed to administer the Federal Quota Program for State Departments of Education, special schools for blind students and students with multiple disabilities, and rehabilitation programs.
