DISCIPLESHIP QUESTIONNAIRE

This questionnaire is designed as an opportunity for each person to reflect upon where they are at as a follower of Jesus.

Responses are anonymous, although you will have the opportunity to share your responses with your Community Pastor, Small Group Leader, or anybody else you decide.

We encourage you to answer honestly, as this will help you to be able to identify your own strength and growth areas. The answers will also help to guide our pastors as they plan how to love and lead our church communities as followers of Jesus.

Wherever you're at on the journey, we're just glad you're on it! There are no right or wrong answers. Our heart is to see Jesus working within your life, and leading you at the pace and in the grace He has for you!

You may find it helpful to complete this questionnaire at a time and place where you can be unrushed and undistracted. We also encourage you to ask the Holy Spirit to speak to you as you reflect upon and answer each question.
