2016 Self-Advocate and Family Member Survey

The Wisconsin Board for People with Developmental Disabilities (BPDD) would like you to please take a few moments of your time to complete the 2016 Self-Advocacy Survey. This survey is for people with disabilities and family members who have participated in a BPDD project/activity (e.g., meeting, conference, community conversation) during the past year.
