Provider Survey Spring 2016 |
Welcome to The Change Foundation’s health care provider survey!
A key component of The Change Foundation’s Caring Experience project is engaging with providers through workshops and an online survey. The Caring Experience project is central to our new 2015-2020 Strategic Plan, Out of the Shadows, Into the Circle with a strategic goal to improve the experience of family caregivers as they help a family member transition through and interact with Ontario’s health and community care systems.
Through this online survey, we are reaching out to frontline health care providers from a variety of health and community care settings to gain insight on the interactions between providers and family caregivers. The survey also aims to gain a provider perspective on the supports that are currently available for family caregivers, as well as what other resources might be needed for providers and for family caregivers.
For this project, “frontline health care provider” includes any professional in the health or community care sector who interacts with family caregivers on a daily basis.
The survey should not take more than 10 minutes to complete. All answers provided by survey participants will also be kept confidential by The Change Foundation.
