Thunder Bay Community Health Service, Inc. (TBCHS) is a federally qualified health center whose mission is "Caring for the People of Our Communities" and vision is "A Community Partner for Well-being." TBCHS has developed a community needs assessment survey to improve the delivery of health care services, and we invite you to participate!
The purpose of the survey is to identify rural health needs from people who live within the communities we serve. Your valued opinion and viewpoints will assist us in developing strategies that can guide future health planning efforts. The survey results will remain confidential and reported in aggregate form. Survey completion time is estimated at three minutes or less.
