Healthy Montgomery, the County's local health improvement coalition, is asking local public and private dental offices and dental clinics to complete a Community Health Needs Assessment (CHNA) Oral Health Capacity and Demand Environmental Scan survey to assess capacity and demand from organizations in Montgomery County to provide preventive and treatment care for a range of diseases and conditions of the mouth. 

The environmental scan will assist in aligning our efforts and leveraging resources to enhance access to oral health care in Montgomery County.  Survey responses will be aggregated and included in Healthy Montgomery’s CHNA report that will be shared publicly and reside on the Healthy Montgomery website. 

The findings from this environmental scan, along with information from the quantitative and qualitative components of the CHNA will be used to support the priority setting process and development of Montgomery County’s Community Health Improvement Plan (CHIP). 

This environmental scan will collect information on your patient population demographics and access to oral health care; organizational capacity; clinical care and utilization; and workforce infrastructure.

All information collected will be kept confidential. All responses are anonymous. One survey response per dental office/clinic.
