Introduction

The Community Perception Survey covers three main areas:

·       Community perceptions of local issues with alcohol, other drugs and mental health and wellbeing.

·       Strategies to address these issues that would be supported by the community.

·       Use of alcohol and other drugs within the community.

Collecting this information from a broad cross-section of participants can help to identify which particular issues are affecting your community. This information can then help to monitor changes in the community over time, as well as clarify which strategies to address these issues might be more successful. It also allows us to get a better idea of what your community would like to see.

Participation in this survey is completely voluntary and you do not have to answer any questions you do not feel comfortable answering. All of the information you provide is completely anonymous and your responses will be pooled with the responses from all the other people completing the survey. The results will help to improve alcohol, other drug and mental health activities in your community.
Please note that this survey must only be completed by participants over 18 years of age. It should take approximately 15 to 20 minutes to complete.

Thank you for taking the time to complete this survey. Your participation is appreciated.
