About this survey
Equilibrium Healthcare takes mental health seriously. We are conducting this survey to better understand the current mental health needs of our community in order to develop a locally targeted mental health program. Your responses will directly shape our October 2026 campaign, the health services we offer locally, and may be used to help us advocate for improved public healthcare services.
