This survey was created by the Mental Health Task Force of the SWFL Healthcare Coalition to help identify Mental Health resources and gaps related to first responders and healthcare professionals in Southwest Florida.  There may be a resource guide created as a result of this survey, however you will be contacted if prior to anything being shared publically.
