FABM Medical Professionals and Educator Database Survey

FACTS, the Fertility Appreciation Collaborative to Teach the Science, is a non-profit organization dedicated to educating medical professionals about natural or fertility awareness based methods (FABMs) and expanding access to FABM trained medical professionals and educators.

This searchable database will be published on the FACTS website and will be available to medical professionals, residents, students, patients, and the general public. By completing this survey, you agree to have your practice information included in our database. Submissions provided will appear as self-reported, however, the information will be vetted and updated regularly for accuracy and consistency.
