Consent and Authorization Document for Minimal Risk Research


You are being asked to take part in a research study. The purpose of this study is to improve the inclusivity and diversity of library services and spaces for traditionally underrepresented students at the University of Utah Marriott Library. Before you decide it is important for you to understand why the research is being done and what it will involve. Please take time to read the following information carefully. Ask us if there is anything that is not clear or if you would like more information. Take time to decide whether you want to volunteer to take part in this study.

It will take approximately five minutes to complete this study. As part of this study you will be asked to share your contact information if you would like to participate in a focus group about how you use the Marriott Library. If you don’t give us your contact information, all of your responses will be anonymous.  If you do share your name and contact information, we will remove any identifying data before sharing this material with others.

The risks of this study are minimal. You may feel upset thinking about or talking about personal information related to the Marriott Library with other students. These risks are similar to those you experience when discussing such information with others. If you feel upset from this experience, you can tell the researcher, and he/she will tell you about resources available to help.

There are no direct benefits for taking part in this study. However, we hope the information we get from this study may help develop a greater understanding of  how to make the J. Willard Library an even more welcoming space in the future.

Research studies include only people who choose to take part.  You can tell us that you don’t want to be in this study.  You can start the study and then choose to stop the study later.  This will not affect your relationship with the investigator.
