Maplewood Elementary, Maple Valley Schools, Hybrid Parent Survey

Dear Parent or Caregiver,

Your child's school is participating in the "Safe Routes to School" program and wants to learn your thoughts about children walking and biking to school this survey will take about 5-10 minutes to complete. We ask that each family complete only one survey per school your children attend. If more than one child from a school brings a survey home, please fill out the survey only once.

Your feedback is important. However, your participation is completely voluntary. You may refuse to answer any questions. By participating, you indicate your consent to participate in the survey. There are no known risks associated with your participation and it may contribute to a better understanding of issues concerning motorized and nonmotorized transportation to schools. Your child will also fill out a survey. You can request a copy of this for review. If you have concerns about this, please contact your child's school. 

Your responses will be kept confidential and neither your name nor your child's name will be associated with any results.
