Welcome to the Opioid Health Home Application for MaineCare providers.

Opioid Health Homes (OHH) deliver team-based treatment of opioid use disorder including medication, opioid dependency counseling, and comprehensive care management for eligible MaineCare members and uninsured individuals.

More information on the adopted rule can be found in the MaineCare Benefits Manual, Chapter II, Section 93.

OHH Services Description

OHH Core Services include:

• Comprehensive care management and coordination;
• Care plan development to identify health-related social needs and to support individual needs of patients and their families;
• Transition support between inpatient, outpatient, and other care settings;
• Referrals for community resources;
• Substance use education and counseling; and
• Access to recovery-focused services.

Application Process

In addition to this Survey Monkey application, the following documents must be submitted to OHH-Reporting.DHHS@maine.gov for the application to be considered complete.


1) Organization’s most recent Form 990, also known as the Return of Organization Exempt from Income Tax Form, or Form 1120, also known as the U.S. Corporation Income Tax Return Form, dependent on organization type.

2) Once the final application and additional required documents have been received, MaineCare will review and deliver a decision within 30 business days by email.

3) Within 30 business days of approval, MaineCare will schedule an onsite training to provide an overview of the billing process and the Maine DHHS Value-Based Purchasing Management System (VMS) portal.
