Completing this form is required in order to cancel OR substitute for teachers already registered but no longer attending Laying the Foundation (LTF) training at Bryan Station High School (July 11-14, 2017).
Substitutions:
AdvanceKentucky will formally register the new teacher for LTF.
The substituting teacher will receive email confirmation of his/her registration after we have processed the substitution.
For Cancellations:
Cancellations received forty-five days prior to the first day of training are eligible for 100% credit.
Cancellations received thirty days prior to the first day of training are eligible for 50% credit.
Cancellations made within thirty days of the first day of training will be charged the full registration fee
No-shows will be charged the full registration fee.
