Please complete this survey to be selected to participate in a focus group to discuss a new educational campaign made by the Oregon Health Authority for health care workers, dental workers, and health educators in Oregon. The purpose of this survey is to find a time slot for pre-set focus groups that may work for you.
All focus group participants will receive a $150 incentive via check by mail. Focus Groups will be 90 minutes long. All focus groups will be virtual and will be held via Zoom.
To be eligible to participate in a focus group and receive and incentive you must work in Oregon and have a physical mailing address in Oregon, SW Washington, or Western Idaho (within commuting distance to Oregon).
