1.INFORMED CONSENT LETTER
for
A Relational Model of Discipleship for the Local Church
Research Project

You are invited to be in a research study about a relational model of discipleship for the local church being done by Adam Thornton, a doctoral student from Asbury Theological Seminary. You are invited because you are a member of Dripping Springs Methodist Church or frequent attendee of Dripping Springs Methodist Church or its ministries.

If you agree to be in the study, you will be asked to participate in one or more of the following: an online survey, a focus group, or an interview. You will not receive any compensation for your participation.

It is important that you know these things about your participation in this study:

1. Your identity will remain confidential. The data you provide will be kept in strict confidence and all efforts will be made to insure your privacy. Focus groups are asked to keep the information shared within the group; however, we cannot guarantee they will do so.

2. You will be audio recorded in the focus group and interviews through iPhone audio recording. These recordings will be kept secure by storing them on a hard drive, flash drive, or other electronic storage device.

3. Any data reported in studies or presentations will be grouped across participants. If your comments are included in the report, a random number or random initials will be used.

4. All data from this study will be maintained securely and reviewed confidentially in a private setting until it is destroyed by electronic deletion or shredding in the case of physical materials.

5. The risks associated with participating in this study are minimal. The primary concern is the possibility that information shared within the focus group could be disclosed by a participant. If you are uncomfortable during the study, please reach out to Adam Thornton who can be reached at athornton@dsmethodist.org.

6. The benefits of this study are that the data will be used in the formatting of a possible discipleship model for Dripping Springs Methodist Church and other local churches in the Mid-Texas Annual Conference of the Global Methodist Church, as well as other local churches that belong to other Christian denominations.

7. You may refuse to responsd to any or all of the questions, and you are able to withdraw from the study at any time without penalty.

By signing this consent form, you are acknowledging the following: (1) you have read this form; (2) you are satisfied with your understanding of this form; (3) you are satisfied with your understanding of the research study, as well as its risks and benefits; (4) you are 18 or older; and (5) you want to be in this study.(Required.)
