Dia-Elf Visit Registration

1.Hi! Before we start, we want you to know that your information is confidential and that these visits are intended to be surprise visits for children impacted by any form of diabetes. We want to inspire them to live strong and empowered lives with diabetes through this project. Please type "yes" acknowledging that you stand with us in our mission.
2.Awesome! We can't wait to meet you. Start by telling us your name.
3.Who in the household has diabetes? Please tell us the name(s) and age(s) of all children and adults.
4.Good to know! What is the registered address we should give our elves so they can find your home?
5.This is so exciting! The elves will be making appearances on four different days. Please check off the days your family can available.
6.Please let us know you are okay with the elf's buddy taking pictures/videos during the visit so that we can spread the word about this to the community.
7.Santa has asked all elves to follow national guidelines for social distancing during the surprise visits. Please type "yes" acknowledging that you and your family understand, you will adhere to this during your visit, and that you release Type Zero Foundation Inc. from any claims and demands that may occur during participation in the Dia-Elf Project held between December 13th - December 20th, 2020. You also agree to release Type Zero Foundation Inc. and will hold them harmless from any liability which may arise from incidents or accidents involving your child/children, yourself, and other household members during the visit.
8.Alright, now that we got through the nitty gritty, our last question will be used to give the elves a special heads up before they arrive for your visit.
