Maxwell Unified Local Control and Accountability Plan (LCAP) Parent Survey - Spring 2017

The goal of this survey is to gather information on the design and implementation of the District priorities for the Local Control and Accountability Plan (LCAP).  Each one of the State's eight priorities have been addressed in this survey.
 
Questions 1-3 are optional and you are not required to submit responses if you are not comfortable answering them.  We are asking them only to determine the response percentage of those students that are classified as unduplicated.  Unduplicated students are those that qualify to receive free or reduced meals, a foster child, or an English Learner.
