Guidelines & Criteria

The Mission United Lyft Rideshare Program assists veterans who need transportation assistance for doctor’s appointments, grocery shopping, legal consultation, housing, employment interview, economic stability, etc.

The request for transportation must be on behalf of a veteran that resides in Orange, Osceola, or Seminole County in Florida.

The veteran must have the Lyft App on their phone and have a current Lyft account with a valid payment option on the account.

All required documentation (Photo ID and DD214) must be uploaded with this fully completed transportation request form. 

The transportation request form and required documentation must be submitted no less than two business days before when the ride will be needed. Requests for weekend transportation must be submitted by no later than 4:00 p.m. on Wednesday before the weekend.

Veterans whose requests have been approved will receive a rideshare code valued at $25 maximum for a one-way ride (or a maximum of two $25 rideshare codes for a round-trip). Veterans must be able to pay for any overages beyond the $25 rideshare limit.
