Welcome to the Child Anxiety Survey!

This survey is part of the University of Reading's 'Parenting and Child Anxiety' project. We are hoping to learn more about parents' thoughts and experiences with their child as well as their child's confidence and anxiety levels.

The survey takes around 20 minutes to complete and all your answers will be kept confidential. At the end of the survey you will be asked whether you would like to enter the prize draw to win £50 of vouchers for Debenhams. If you would like to be entered you will need to provide your email address. This means we can identify which responses are yours but once the data has been downloaded, your email address will be removed and stored separately in a password protected file. If you do not enter your email address at any point your answers will be completely anonymous. 

Both parents' experiences and perspectives are important and so we would like both mums and dads to complete the survey independently. You don't need to do them at the same time, you can both access this survey separately at a time that suits you. Single parents families are also welcome to take part.

Electronic data will be stored on a secure server and destroyed when no longer needed. Cookies, personal data stored by your web browser, are not used in this survey.

Taking part in this survey is purely voluntary. You can withdraw from the study at any point, even after submitting your answers, if you keep a note of your ID number. You can do this by using the contact details below.
