NOTE: THIS APPLICATION MUST BE SUBMITTED BY A DISTRICT ADMINISTRATOR.
The school district employing the social worker working toward school social worker credentials must complete and submit this application for reimbursement. Reimbursement will be made to the school district, not individual social workers.

The maximum reimbursement benefit is $1,000 per credit hour for up to 9 hours of school social worker courses if all grant requirements are met. Tuition reimbursement does not apply to repeat courses.

Before beginning the application, please make sure you have the following documentation available:

- A screenshot of the School Social Worker Permit from MOECS
- The tuition receipt from a university with an approved school social worker program (including social worker’s name, semester, course number and name, number of credits, tuition broken out from fees, and proof of payment) in a PDF format.
