Thank you for participating in this survey. The Milwaukee Jewish Federation is exploring social services and how they are funded in the Milwaukee-area Jewish community. For this purpose, we define "social services" as help for any issue that affects day-to-day functioning of individuals and/or families including poverty, financial crises, hunger, disability, aging, mental health, housing, etc. By completing this survey, you will help the Federation learn more about our community's needs and use of social services.

The survey should take about 5-10 minutes to complete and your answers are completely anonymous. In the survey we do not ask for contact information and responses are not tracked or tied to any identifying information. If you have any questions, please feel free to contact us at

As an added thank you, at the end of the survey you will have an opportunity to enter a drawing for an award of $100. There will be five winners chosen and each will receive the $100 award. As your answers to this survey are anonymous you will need to click an external link to be entered into the drawing. Information collected from this link will not be connected to your survey responses.
