Structure
:
  • 4 Instructional & Informational Webinars (90 minutes each)
  • 2 Small Group Coaching Calls for Questions, Support and Technical Assistance (60 minutes each)
  • 2 Round Table Discussion Sessions (60 minutes each) on Specific Topics such as:
    • Effective Engagement and Communication Strategies: Using MI and SDM
    • Monitoring Patient Medication Adherence and Organizational Progress
  • Key Resources: Medication Adherence Toolkit and MA-OSA along with a variety of other supporting tools and resources associated with the assessment process and each domain area

Learning Objectives:
  • Conduct an analysis of their strengths and gaps related to best practices in medication adherence through completion of a Medication Adherence Organizational Self-Assessment
  • Become aware of the importance of the role of assessment, treatment planning, prescriber interventions, organizational supports and medication monitoring in promoting adherence and improved health outcomes
  • Gain skills and knowledge related to effective engagement and communication skills, improved assessment practices, policies and procedures, development of collaborative treatment plans, and use of best practice interventions to address barriers and monitor patient adherence to medications
  • Apply the skills and knowledge gained to the development and implementation of new or revised assessment process workflows that incorporate key steps and strategies
  • Collect data to track and analyze information related to improvement efforts and measure patient rates of adherence
