Informed Consent - IRB# 61-2017/18

This study is being conducted by researchers Elizabeth Nakamura, Danielle Quan, and Jamie Reynar as part of the requirement for the Doctor of Physical Therapy degree at Mount Saint Mary’s University – Los Angeles. It is designed to collect data and determine trends in satisfaction among individuals who have used exoskeleton technology. The research will add to the limited literature available and be helpful to clinicians, patients, and manufacturers.

Your participation in this study is voluntary. You may choose to not answer any of the questions or withdraw from this study at any time without penalty. Your decision will not change any present or future relationship with Mount Saint Mary’s University. You will be asked to take a one-time online survey, which should take no more than fifteen to twenty minutes. You will not receive monetary compensation for participation in this study. No personal identifiers will be collected or provided to the researchers. The cumulative results of this study will be published but the names or identity of subjects will not be known. All data and documentation collected as part of the project will be kept on file by the researchers at the conclusion of the study. Although every reasonable effort has been taken, confidentiality during actual Internet communication procedures cannot be guaranteed.
