Please take this brief survey concerning Florida HOA's using Toxic Chemicals including Glyphosate in Our Communities!

1.Are you aware the chemical glyphosate used by landscaping contractors and in agriculture is known to be carcinogenic and is linked to cancer, reproductive and neurological harm according to numerous scientific and medical studies? and are you aware of the ongoing cancer related class action lawsuits including non-Hodgkin's Lymphoma caused by exposure to glyphosate-based weed killer?
2.Do you agree HOA’s should ban the use of toxic chemicals including glyphosate in common areas, around homes and on private lawns?

Do you also agree toxic chemicals should not be used where children play and people and pets walk?

Do you agree it is unacceptable HOA's and contractors refuse to disclose the use of toxic chemicals in our communities including name and type of chemicals used?

Do you also agree it is unacceptable HOA's and contractors refuse to give residents warning before and after chemical applications so direct exposure to people, pets and children can be avoided?
3.Do you agree toxic chemicals including glyphosate-based weedkiller should not be applied in the water or in or around community lakes where children and pets play and families fish? Do you also agree it is unacceptable to apply toxic chemicals in the lakes in direct contact with wildlife including endangered and threatened species?
4.Are you aware contractors are applying toxic glyphosate-based weed killers directly in community lakes resulting in fish suffocation killing fish? and are you aware the natural green plant life in the lakes and beneath the water is being completely destroyed turning to brown sludge?
5.Do you agree the natural green plant life and wildlife including numerous bird species in community lakes is attractive? and do you agree community lakes fried from chemicals with burned yellow and brown grass around the perimeter and brown decaying sludge with no more fish in the water in unattractive?
6.Do you agree HOA's should protect wildlife, people and pets at community lakes by banning the use of toxic chemicals including glyphosate in and around the lakes?
7.Do you agree it is unacceptable for contractors to apply toxic chemicals in a manner that violates federal law including spraying in the wind causing drift into lakes, surrounding properties and around people in the area?
8.Are you aware you can opt out from receiving chemical applications on your property?

Are you also aware you can reject mulch applications containing toxic chemicals and synthetic dyes known to cause cancer and reproductive harm, including mulch with Proposition 65 warnings?

Are you aware you can receive prior notification of pesticide applications directly from licensed contractors by completing an application to be placed on the FL State registry?

Are you aware licensed contractors are required to comply with State law and provide prior notification of pesticide applications to all persons included on the registry?

Are you aware you can request an investigation with the State Agriculture Department if contractors fail to provide notice and or improperly apply or misuse chemical pesticides, herbicides and insecticides in violation of law?
9.Are you resident or visitor at an HOA or resident or visitor at Keys Gate HOA in Homestead, Florida?
10.Have you or anyone in your household or anyone you know in your community been diagnosed with any of the following medical conditions?

Parkinson's Disease

Dementia

Non-Hodgkin Lymphoma (NHL) or similar cancers including:

Chronic Lymphocytic Leukemia (CLL)
Diffuse Large B Cell

Small Lymphocytic Leukemia (SLL)

Hairy Cell Leukemia

Mantle Cell Leukemia

Or have you or anyone you know had any adverse reactions after pesticide or chemical exposure at your HOA including any of the following symptoms: nausea, vomiting, stomach pain, headaches, dizziness, skin rash, nasal or eye irritation, trouble breathing or shortness of breath or any other ill effects?
