2023 People With disabilities WA Member Survey

Welcome to our Annual Member Survey for 2023. 

 

We want to hear from you about how you feel PWdWA is going and how we can improve. In addition, we want to know what systemic issues are important to you.  We also want to check your details to ensure our information is current.  All the information you give us is treated with strict confidentiality.

 

The survey will take about 15 minutes, you can take a break from completing the survey and come back to it at a later time.

 

If you would prefer for Vanessa to read the questions and scribe your answers, please call her at 9420 7279 ext 717 (Monday to Thursday). 

 

By completing and returning the survey before Wednesday, 12 April 2023, you will go in the draw for a $250.00 gift voucher.  This will be drawn on Thursday, 13 April 2023.
