The National Disability Inclusion Strategy (NDIS) 2017 - 2021
 is the Government’s plan to improve the lives of persons with disabilities. There are 114 actions under this plan to help people with disabilities. The actions cover many topics and include helping people with disabilities go to school, go to work, live in the community and enjoy the best health possible.

As part of this plan, it was agreed that the Government would look at the actions half way through the plan’s lifetime. The Department of Justice and Equality is asking those persons with disabilities and disability organisations to answer some questions to help write a plan for the NDIS for 2020 and 2021.  The following survey asks 6 questions:
