Survey on Sleep Quality and Food Security

Implied Consent

Greetings
My name is Theresa Gibison, and I am in a bachelor's program at Wilmington University. I am studying the association between food security and sleep habits. I would appreciate your help with my study by taking a short survey about this important topic. The survey should not take more than 10 minutes to finish.
The study results will be used for class purposes, but your name and anything that might identify you will not be made public.
Participating in the survey is voluntary. You may skip any question you do not want to answer and may stop the survey at any time. I hope you will take a few minutes to answer all the questions so we can better understand food security and sleep habits.
If you have any questions, please feel free to contact me at tgibison001@my.wilmu.edu or my research advisor, Professor Cheeseman, at kelly.a.cheeseman@wilmu.edu.
Thank you, Theresa Gibison

Food Security
The next set of questions asks about access to food over the past 12 months, since April of last year, and whether you were able to afford the food you needed. Everyone's situation is different, and many people experience changes in their ability to get the food they need. These questions are a standard part of health and well-being research and are asked of people from all backgrounds. Your responses will help us better understand how food access relates to sleep habits and overall well-being.
There are no right or wrong answers, and your responses are anonymous.
Please answer as honestly as you can based on your experiences during the past 12 months. Some questions may refer to " You or other adults in your household." If you live alone, answer based on your own experience. If you live with others and share food or meals, answer based on your household as a whole.
4.The food that we bought just didn't last, and we didn't have the money to get more.(Required.)
5.We couldn't afford to eat balanced meals.
6.In the last 12 months, did you or other adults in your household ever cut the size of your meals or skip meals because there wasn't enough money for food?
7.If"Yes" to question 6: How often did this happen?
8.In the last 12 months, did you ever eat less than you felt you should because there wasn't enough money for food?
9.In the last 12 months, were you ever hungry but didn't eat because there wasn't enough money for food?
