Application for Learning Essential Approaches to Palliative Care Mini (LEAP Mini) In-Person Free Courses: Fall 2025

Champlain Hospice Palliative Care Program

* These courses are only open to healthcare team members who work in the Champlain region of Eastern Ontario. This includes any health care professional (e.g., physician, nurse, pharmacist, social worker, etc.) who would like to complete training in palliative care and who provides care for patients with life-threatening and progressive life-limiting illnesses.

* We are not accepting Personal Support Workers or unpaid caregivers into this course.

* Normally, LEAP training costs registrants around $165-$250, but the registration fee is being subsidized for these three courses.

* Participation and attendance for the full day is mandatory.

* Registrants must complete the pre-course work before the in-person workshop day.

* If you are accepted into the course, you must create a Pallium Canada account (if you do not already have one) and register using the promo code we would provide you with.

Registrants must complete all of the online pre-course work before the full day in-person workshop:
6.I am able to complete all of the pre-course work prior to the workshop and am able to fully attend the in-person full-day workshop. This is a requirement in order to be registered in the course and to receive a certificate of completion.(Required.)
If you are accepted into a course, you will receive an email before the start of the training to confirm your spot in the course. The email will let you know which workshop date and location you were accepted for. Upon acceptance, you'll need to create a Pallium Canada account (if you don't already have one) and register using the promo code we provide.

Please provide at least 24 hours' notice if you need to cancel. Failure to notify us within this timeframe will result in your future applications being waitlisted for the following year.

Failure to attend sessions that you have registered for prevents others from the opportunity to attend. It is important that if you cannot make a session that you have registered for that you take the appropriate steps, so that someone on the waiting list could fill your seat.
