Welcome to the DOH's Healthy Lifestyle Post Survey

The objective of the following survey is to know the changes of behavior and knowledge about your current diet and exercise practices to help the Department of Health to improve and change the activities that has been impleted through Healthy lifestyle campaign (HLC). This survey will collect your response anonymously therefore your answers will be confidential. Your honest answers will help us develop activities that best meet your interest and needs. The survey is estimated to take only about 10 minutes to complete. All but one question are multiple choice. Please write your answers in English only.
