Directions
1. Before completing the online application please access the following links and gather the necessary information:
- Tiered Fidelity Inventory completed for all 3 tiers
- Tiered Fidelity Inventory Online Scoring Tool (can be used for ease of scoring. Action Plan not required)
- 9 weeks Office Discipline Referral data completed for each 9 weeks for the current year
2. To be considered for REACH MS SWPBIS Model Site, complete all fields on this online application with your school’s information.
3. After submitting the application, notify REACH MS that your school has submitted the application for Model Site. Email REACH MS at: www.reachms@usm.edu
