Who is this survey for?
Families of children/young people in Essex who:

  • are awaiting a Needs Assessment for an Education, Health and Care Plan (EHCP)
  • have recently had an EHCP Needs Assessment carried out

What do we want to know?
This short survey will ask questions about:

  • some general information (area of Essex you live in, type of education setting)
  • waiting times for EHCP Needs Assessments
  • communication with Essex County Council (ECC)

Why are we asking for this information?
We would like to understand how many families:

  • are experiencing long waiting times for an EHCP Needs Assessment.
  • have children/young people currently not able to attend their education setting full-time
  • have children/young people who are in a transition year*

*starting primary school for the first time in September 2025, in Year 6 and starting secondary in September 2025, moving into Post 16 education or Post 19 education.

* 1. By providing consent, you are agreeing that personal information about you will be collected anonymously stored securely and will not identify anyone personally. The anonymised data will be used and shared with relevant organisations for the purpose of improving SEND provision within Essex and England but will not identify anyone personally.

Once consent has been provided you have the right to withdraw your views and your views will be removed, however where data has been anonymised and amalgamated in certain circumstances it may not be able to be excluded.

If you are completing this form on behalf of someone else, please ensure you have the appropriate consent to do so.
