Coach Certification Message / Disclaimer

On behalf of AMHA I would like to thank each of you for considering and/or applying to coach one of our C or Initiation teams for next season. The time and commitment you have already given to the game is greatly appreciated.

This application form is for C Level coaching positions only for U6 - U9 (H1 - H4), U11 (Atom), U13 (Pee Wee), U15 (Bantam), U18 (Midget), and U21 (Juvenile levels. If you are interested in applying for one of our REP teams please fill out the REP application form.

Prior to applying for one of the teams please read the Coach Certification Requirements outlined below for the level and position you are interested in. If you don't yet have the certification level necessary at this time that is ok but be aware that you will need to attend the appropriate clinic in the fall to be considered for one of the positions.
 
Division: U6-U9 (H1 - H4)
Head Coach: Coach 1 Certification AND prior to season start Respect in Sport, CATT, Criminal Record Check  
Assistant Coach: Coach 1 Certification AND prior to season start Respect in Sport, CATT, Criminal Record Check

Division:  U11 - U15 (Atom/Peewee/Bantam)
Head Coach: Coach 2 Certification, Checking Skills Certified AND prior to season start Respect in Sport, CATT, Criminal Record Check 
Assistant Coach: Coach 2 Certification AND prior to season start Respect in Sport, CATT, Criminal Record Check

Division: U18 & 21 Midget & Juvi)
Head Coach: Coach 2 Certification AND prior to season start Respect in Sport, CATT, Criminal Record Check  
Assistant Coach: Coach 2 Certification AND prior to season start Respect in Sport, CATT, Criminal Record Check
