From the 1st of September 2015 the Mid Yorkshire NHS Trust started charging disabled blue badge holders to park at its hospitals. The aim of this questionnaire is to consider the impact of the blue badge parking charges on disabled patients, carers and visitors.

We will always take necessary steps to ensure that your information is protected and treated securely. Any details you give will be held in accordance with the Data Protection Act 1998 and our data protection policy.
