Information
Thank you for your interest in becoming a 100% vital and thriving congregation. Even if you are vital and thriving now, engaging in an intentional process every 5-7 years is a best practice to ensure continued future vitality. This application is designed to collect important information that allows the church to learn about itself in preparation for making decisions about taking action to improve ministry.
This application will need to be reviewed and verified by the team named in the application so that all are in agreement with what is being reported. The application deadline is February 15.
Each congregation that completes an application will be invited to participate in one of the congregational vitality initiatives: Catalyst or Readiness. Decisions will be made by March 4.

This application will take approximately 1 hour to complete once you have your Readiness 360 scores. To better prepare you to complete this in one sitting, use the worksheet located on this page to help. This application requires you to complete and upload:
Readiness 360 results - request your information by Nov 1, 2023, using the Intent to Apply form (BWC | PDC). This congregation-wide survey takes approximately 3-4 weeks to implement.
