A health assessment is a snapshot of the health of a community at a point in time. It describes a variety of health topics, as well as social and economic factors that influence health. Assessing the health of a community or region is an ongoing process that involves monitoring population health, and measuring progress toward improving it. We need your help to assess progress and identify areas for improvement.
