Assessing the health of a community or region is an ongoing process that involves monitoring population health, and measuring progress toward improving it. We need your help to assess progress and identify areas for improvement.
Please take a few moments to provide your answers to four questions that will help us improve progress toward a healthier Crook County and Central Oregon. We are grateful for your input!

* 1. What assets and resources do we have that can be used to improve community health?

* 2. What are the major health concerns in the community?

* 3. What do you consider barriers to health and health care?

* 4. How is quality of life perceived in our community?
