Receive gift cards for completing surveys and attending community workshops.

Provide your opinion about programs.  We want to hear about challenges in your community that may prevent families from breastfeeding, safe sleep, or attending well child visits.

The Ambassador should be comfortable sharing their experience and giving honest feedback about the Mother Infant Health & Equity Improvement Plan.  The Ambassador should attend at least 2 community workshops and share feedback through surveys.  The Ambassador must be 13 years or older.

Transportation to workshops will be provided if needed.  Access to email or texting is helpful for surveys, but not required.  We will provide paper copies if needed.
