How do I answer if I'm a caregiver for a person with mental illness who uses services and I do not use services myselfPlease answer based on your experience with services.  Skip or select "N/A" if a question is not applicable.  Please answer as many questions as you can.  For any questions you are answering from the perspective of the person you provide care for, please note that the comment boxes ("Caregiver answering for consumer.").
What is this survey for  This survey reaches out to the community and collects input.   Community outreach is required by the  the Mental Health Services Act (MHSA).  Want to know more?  You can read or review the Mental Health Services Act or review information from MHSOAC.
Why are some programs listed here and not others?   That can be confusing!  This survey covers programs that are funded through Mental Health Services Act (MHSA).
Are there other ways I can provide feedback on MHSA?  Yes.  Along with completing this survey, please considering coming to one of four community outreach meetings.
