Who Should Complete this Survey?
The survey seeks responses from physicians (DO or MD) and nurse practitioners (NP) (collectively “providers”) regardless of practice size or ownership. For purposes of this survey, a “practice” consists of one or more providers that may operate independently or be employed by a hospital. The MHCC encourages all providers in the practice to respond to the survey. Note, demographic information is used to identify practice information to conduct practice level analyses.
