Thank you for your interest in taking the Oxford Food Governance Group's 'Labels and Apps' survey.

This brief (~10 minute) survey includes multiple choice, rating, and open-ended questions; the questions are designed to assess your preferences and utilization of food-related labels and apps.

The information you provide will remain confidential, and will be accessible only to researchers at the Oxford Food Governance Group. If you would like to participate in follow-up interviews, please provide us with your name and email address; however, if you do not wish to participate in follow-up interviews, you may choose to remain anonymous. You do *not* have to provide your name and email address to take this survey. You may quit this survey at any point you choose, and are not obligated to complete it. The researchers will use the information you provide as part of a larger study that examines consumers' use of information and communication technologies in food-related decision-making.

Please note that the study has been reviewed and approved by the University of Oxford's Central University Research Ethics Committee. If you have a concern about any aspect of this project, please contact the Principal Investigator, Professor Stanley Ulijaszek (Stanley.Ulijaszek@anthro.ox.ac.uk), who will do his best to answer your query. If you remain unhappy and wish to make a formal complaint, please contact the Chair of the University of Oxford’s Social Sciences Interdivisional Research Ethics Committee (ethics@socsci.ox.ac.uk), who will then direct your complaint to the appropriate body.

By pressing 'next' you will confirm that you have read and understood this information, and that you consent to take part in this study.
