Volunteer and Coach of the year Nomination Form

Any member or individual (in good standing) involved with NSMHA may be nominated.
Main criteria for selection:
(i) could be presented to a head coach, assistant coach, manager, official, executive member, board member, etc.
(ii) individual that has gone well beyond the “call of duty” for the association or team.
