I understand by filling out this form, I am committing to a sponsorship for the 2022 Martini Masquerade on Saturday, March 19th, 2022 at the Grand Hotel and that refunds will not be made.

No payment by January 31st, will result in loss of sponsorship.

In the event of unforeseen circumstances (including COVID and weather) resulting in a postponement or cancellation of the fundraiser, you will be given the opportunity to transfer your sponsorship to a monetary donation or transfer it to the next fundraiser.
