Review of support and information needs in people diagnosed with breast cancer
Dear Participant,
Thank you for agreeing to take part in this survey as a part of a collaboration between Synthesis Clinic and NCIO (National Centre for Integrative Oncology). We really appreciate your time, support and contribution to this work. The full project name is 'Exploring unmet needs in people with solid tumours and the impact of integrative oncology: a mixed methods study', and this survey has received ethics committee approval with ref SYN30525. We are interested in understanding the holistic needs of people diagnosed with breast cancer and how these needs are met in standard oncological/hospital care. The collated anonymised survey results are planned for presentation and where appropriate, publication, to help the oncology community further understand and plan for needs of patients impacted by breast cancer.

Study consent statement:
My participation in this study is voluntary and I understand that I can withdraw from the study at any point without giving a reason. I have read the information provided, and I understand what is expected of me. I have been given the opportunity to ask questions, and they have been answered to my satisfaction. I understand what data will be collected about me, how it will be used and kept and who it will be shared with. I understand that the data collected from me will be securely kept in de-identified form and will be used for publications and presentations with the opportunity to be re-used by others.
