Serve Indiana Certifications and Credentialing: AmeriCorps Application Supplement

Please use this form to provide information to Serve Indiana on the certifications and credentialing your program will be offering to AmeriCorps members. Programs who are new or re-competes will complete this form as a supplement to their AmeriCorps grant application.
1.Please list your program name as it appears on your grant application.(Required.)
2.Please list any and all certifications or credentials your program will offer to members in the program year for which you are applying. Please provide direct links for the certifications offered, as well as a description of each certification. At minimum, your program must offer a Level 1 Certification to all full-time members. For a description of Level 1 Certifications and examples, please see the Certifications and Credentialing Checklist and List of Example Level 1 Certifications on the Serve Indiana website.(Required.)
