Medical Director Meet and Greet Request

Please give us up to 5 dates and times for request...we'll get those times in front of the MDs, and coodinate getting them in front of you via ZOOM.

Please note: Meetings by ZOOM are preferable as they allow our MDs more time in front of the crews, and give them the flexibilty to visit multiple services in one evening. 
If you would like to schedule an in-person meetings, please make a note of that in the "Comments" section below.  In-person requests must be made at least 3 months in advance, and cannot be guaranteed.
