The Morris County Collaborative invites you to participate in a community needs and resources assessment. The benefit of this assessment is that you will be helping the Morris County Collaborative to better understand our community needs, the resources available, and the services that are still needed. This information will help us to work together to improve services that are needed the most.

Your involvement is voluntary, so you may choose to complete this survey or not. Please feel free to ask the person who gave you the link to this survey any questions that you may have about it. They will be happy to explain anything in greater detail.

All information will be kept anonymous. This means your name will not appear anywhere and no one will know about your specific answers. We will not share your name with anyone else about how you answered this survey. There are no risks to you for participating in this study, and no penalty for not participating. Services to you will not be affected in any way if you choose to participate or not. If you do not wish to continue, you have the right to stop at any time.

We are interested in learning more about your family’s resources and needs. You will be asked to answer questions that include education, employment, housing, healthcare, and other basic needs.
