SNF Medicaid Survey August 2024

We are working to identify our members’ issues related to the Medicaid application process. This survey will help inform our efforts with DSHS and the Legislature. This is a snapshot in time—please provide us with information on your experiences for the period of July 1-31, 2024. We are collecting information on individual facilities; if you are submitting on behalf of a group of facilities, please complete the survey for each. Individual responses will not be shared publicly.
